Collection and Use of Personal Information
We may collect the following personal information from you:

The primary reason we collect health information is so a clinician can treat you. This information is ONLY shared with the clinician treating you. To provide safe treatment, you agree to supply them with that information and grant access to past Zipnosis visits you’ve completed. They do not have access to any other medical record information or your primary care physician.
Info Obtained from Third Parties
If you provide us personal information about others, or if others give us your information, we will only use that information for the specific reason for which it was provided to us.
Reports created for other adults or legal dependents that you elect to add to your account are only accessible through the primary account holder’s username and password. Minors are not allowed to complete a Zipnosis visit without parental help.
We may provide your personal information to companies that provide services to help us with our business activities such as processing your order or offering customer service. These companies are authorized to use your personal information only as necessary to provide these services to us.
We may also disclose your personal information:
- As required by law such as to comply with a subpoena or similar legal process.
- When we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request.

- Health information with the clinician treating you.
- To facilitate convenient, safe and affordable health care.
The Bottom Line: Only you and the clinician treating you know about your health condition.
Cookies and Other Tracking
We use another company to place cookies on your computer to collect non-personal information to compile aggregated statistics for us about visitors to our site. Our third party partners employ clear gifs (a.k.a. Web Beacons/Web Bugs), images, and scripts that help them better manage content on our site. We do not tie the information gathered to our Customers’ or Users’ personal information.
Third Party E-Commerce Provider
Our shopping cart is hosted by our e-commerce solutions provider. They host our ordering system, and collect your billing information directly from you for the purpose of processing your order. Our company does not use this information for any other purpose.
Links to Other Web Sites
When we collect personal information directly from you and when you enter sensitive information (such as a credit card number) on our order forms, we encrypt the transmission of that information using secure socket layer technology (SSL). We follow generally accepted industry standards to protect the personal information submitted to us, both during transmission and once we receive it. No method of transmission over the Internet, or method of electronic storage, is 100% secure, however. Therefore we cannot guarantee its absolute security. Additional Policy Information
Our Web site includes Widgets, which are interactive mini-programs that run on our site to provide specific services from another company (e.g. displaying the news, opinions, music, etc). Personal information, such as your email address, may be collected through the Widget.
Updating/Accessing/Amending/Correcting Personal Information
If your personal information changes, or if you no longer desire our service, you may update or delete it by making the change on your Account Settings page. If you wish to deactivate your account, please contact us at firstname.lastname@example.org
We will retain your information for as long as your account is active or as needed to provide you services. If you wish to cancel your account or request that we no longer use your information to provide you services contact us at email@example.com. We will retain and use your information as necessary to comply with our legal obligations, resolve disputes, and enforce our agreements.
Notification: Email and Text Messages
You must provide your email address and phone number when creating an account so we can notify you when your clinician has responded. If you elect to receive a text message, we share your phone number with a 3rd party vendor that sends text messages. We share no health or demographic data with this company.
Pharmacy and Prescription Routing
If you get a prescription, you can choose the pharmacy where you want to pick it up. To generate the list of pharmacies we may use two pieces of information:
- A specific address
- Your current location (GPS equipped devices only)
In both cases you must agree to let us use this information. We do not send the pharmacy any information about your current location or home address. We do not share your demographic or health data with drug companies or pharmacies.
Every employee at Zipnosis has completed HIPAA training and job-specific training on how to protect and respect your health information. We have clear policies in place in the event of a privacy issue, so we can react quickly and resolve the issue appropriately.
As our business grows, we’ll continue to provide you more services. If we ever change how we use your information, you’ll find it here. All the data we collect via our website is audited internally to ensure our system is working correctly. It also helps us troubleshoot a problem for you or your clinician. This information is accessed only authorized Zipnosis employees.
